LI Autism Fair & Conference: Vendor Form

                                      Vendor Table Application:
  ** Use this order form if you are a Not For profit group or company **

ALL VENDORS MUST BE APPROVED BEFORE PURCHASE OF A VENDORS TABLE.
To be approved please fill out the appropriate form below.

Vendor setup is Saturday Morning 7am-8:30am. The Vendor Room will
be open from 8:30am-6:30pm Saturday & Sunday. Please ship any materials
directly to:

Please be sure to label all boxes sent "For Autism Fair".

We are expecting 1000-2000 people to visit the Vendor Room throughout
the weekend.

Each Vendor will recieve:
One 6 x 2.4 ft table with tablecloth
Two chairs
AC power
Name and link on website
Name listed on poster and program
Two Sports Plus wristbands
Two LIAF&C Programs
